Well, I haven't been to one in a very long time. But, when I used to go to restaurants, I often left higher tips if the waitress was cute and the service was good. I don't know what percentage I left as tips, but I remember when I left a tip that was higher than the cost of the food itself once. The bill for the food was a little more than $9.00 (can't remember the exact price). I left a $10.00 tip. That was the only time I left a tip higher than the cost of the food, but I remember leaving fairly decent tips before.
20% for good service. Much less if the service is not good.
20 to 25%
depends on how much wine I'd had with my meal....hic
We give 10% as restaurants in England are more expensive than America
16% on average.
I usually tip around 30% because I was a food server once and I know how hard they work for the money.
20% unless they're really great. More, if they are.
20, since 15 is hard to figure out so whenever I try it I end up either massively over or under tipping

I am very generous with tipping food servers.
